Tummy Tuck – treatment, procedures and outcomes

Tummy tucks are best performed when a patient is fit and healthy. No special diet or exercise programme is required prior to an abdominoplasty, but you should be close to your ideal weight. You will be admitted on the day of surgery, and your length of stay depends on which procedure is done. With a full abdominoplasty you will have some dressings on your tummy and usually some drains (plastic tubes attached to suction bottles). The drains will usually be removed before you go home. You will be given instructions about your dressings and stitches when you go home. I will use dissolving stitches. Post-operative pain in these procedures is easily controlled. Patients will be mobile from day one and should be back to full exercise within six weeks. Patients are recommended to take at least four weeks off work immediately after the operation in order to ensure they recuperate fully. These timings are approximate and depend on what exactly is done; also some people recover quicker than others.

All abdominoplasties result in some scarring although the nature of the scars will depend on the technique that has been used. Scars tend to be quite red and raised in the first six weeks, changing over next six months or so and then fading to white. Most patients will form good quality scars over time, but occasionally and unpredictably some patients will get red lumpy scars that do not improve.

LIP FILLERS – FIRST TIME GUIDE

So you are doing it! Congratulations. This is a guide for first time lip filler clients on how things work, what to expect, and how to keep them looking good!

First and foremost, everyone is different, different aesthetic goals, different lip structure, different metabolism, different genetics, different risk factors, etc. Do NOT compare yourself to someone else, you are your own lip, your own architecture, your own volume, be proud of your uniqueness! It’s what makes us different. 

How long does it take?

– 10 minutes of topical numbing 

– 10 minutes of procedure

Is it painful?

– Most clients have minimal discomfort, on a scale of 1-10, they are a 2-3. This is less painful than laser hair-removal, thanks to our unique numbing technique!

How swollen will I be?

– Everyone is different, some people swell up while others do not, swelling depends on multiple factors. Just because you didn’t swell one time, does not mean you will not swell the next time you do a treatment. Sometimes an anti-swelling medication will be given after your injection. I can address this at the time of the visit. Major swelling typically lasts less than 4 days on average, while minor residual swelling can last 14-21 days. 

What about bruises?

– Some people are pre-disposed to bruise, avoiding vitamins, alcohol, and ibuprofen type products can decrease the chances of a bruise. Bruising comes in three sizes: Small (like the tip of a needle), medium (like the back of pencil eraser or two pencils erasers), and large (gum-ball sized). Depending on the type of bruise they can last 2-3 days or up to 2 weeks respectively.  Arnika pills can be purchased during your visit that can help heal bruises. From time to time a laser can be used to speed up the process if warranted for specific cases. 

How long does it last?

– Typically lip injections last 6 months depending on multiple factors including product used. I’ve seen people that have had fillers lasting as little as 2 months and as much as 1 year. 

Will I have to keep on doing the procedure?

No! You can stop doing your lips any time and they will slowly return to near normal. The skin will not “stretch out”. Some people do their lips every 4 months to keep them extra juicy, while others elect to do them every 6-8 months. This is a personal preference and will need to be decided along the way. 

I don’t know what product to use.

– I typically recommend the juvederm line of products there are two main types for lip volume, juvederm ultra plus xc and juvederm ultra. Depending on your aesthetic goals and lip structure we can select what is right for you. New Generation Juvederms include volbella (typically for older clients) and vollure. These will be discussed in person if necessary for your treatment.

I’ve heard about people having bumps…

– Unfortunately bumps do happen! If a doctor tells you that bumps never happen, be cautious, they do happen but it is not very common. Most bumps are small resolving bruises or some of the product. Usually you can massage them out after 2 weeks time, otherwise small droplet injections of a dissolver can be used in rare-instances to help get rid of these bumps

It looks un-even….

– This is rather common! Not to worry, usually swelling accounts for the majority of this, swelling can be worse on one side than the other, let all the swelling come down (7-14 days in most cases) and then re-evaluate the symmetry, 95% of the time this self resolves. 

When can I go back to normal activities?

– After about 24hrs you can resume normal activities

Can I fly after the procedure?

– Yes, but keep in mind that flying makes you dehydrated so try to avoid salty foods and drink tons of water. 

When Can I work out?

– You can return to normal activities the next morning, avoid rubbing your lips vigorously, eating very hot foods, and chew carefully the day of the procedure.

What can I do to keep them looking great?

– I recommend a topical vitamin C based cream from skinceuticals for adequate hydration, drink 8 glasses of water a day, and avoid repetitively biting or “mashing” your lips together as this makes the product dissolve faster. 

What happens in a tummy tuck procedure?

The most common form of tummy tuck involves a horizontal incision near the pubic hairline, so swimwear or undergarments can hide the resulting scar. If there is excess skin above the navel, an incision around the navel may be required. Sometimes, the surgery involves liposuction to remove pockets of excess fat from specific areas of the body. Diastasis is a term used to describe the separation of the two-paired abdominal muscles that make up the front of the abdomen, which is often caused by multiple pregnancies. Abdominoplasty surgery, commonly known as a tummy tuck, is an operation to remove extra skin, scars, stretch marks and fat from the tummy, and sometimes to tighten the tummy muscles. However you could also consider the following. Having liposuction (where fat is sucked out through a tube inserted into small cuts) before or after the tummy tuck, to thin the abdominal wall. This is helpful but the results can be unpredictable. Liposuction alone will have less effect than a tummy tuck. Having an extended tummy tuck, where extra skin and fat are removed from the lower back and above the hips. With this surgery, the scar goes around onto the lower back. An endoscopic tummy tuck, where the tummy muscles are tightened through a short sideways cut above the pubic hair. Skin is not removed, but liposuction can be carried out. An apronectomy, which is a modified mini tummy tuck for if you have a lot of skin and fat hanging down over the pubic area. Here, only the extra skin and fat are removed. This leaves a long, sideways scar. If you have scars from previous operations, the usual tummy tuck may be modified to suit the scars.

Mini-Tummy Tuck: Improves the Lower Belly with Less Extensive Surgery

Are you mainly bothered by a “pooch” or loose skin and stretch marks below the belly button? A mini-tummy tuck may be a good option for you. This technique involves a single incision made just above the pubic mound, through which your cosmetic surgeon will tighten loose muscles (flattening the “pooch”) and remove excess skin to restore a smooth, flat abdominal wall.

The resulting scar from a mini-tummy tuck is a thin, horizontal line that is typically just a few inches long. Benefits of a mini-tummy tuck include a shorter, less involved procedure, a quicker recovery, and a flatter, slimmer lower abdomen that looks better in swimsuits and fitted clothing.

Classic or “Full” Tummy Tuck: Improves the Upper & Lower Abdomen

Even so, a full tummy tuck scar can usually be hidden beneath a swimsuit bottom. Benefits of a full tummy tuck include a much flatter, smoother abdomen, reduced appearance of stretch marks, and enhanced comfort in pants and skirts. You will also find that form-fitting dresses and swimsuits have a more flattering fit.

Extended Tummy Tuck: Improves the Upper & Lower Abdomen and Flanks

Have you lost a lot of weight and now have excess, sagging skin hanging around the front half of your body? Or, have you noticed significant laxity in your abdominal muscles following multiple pregnancies? An extended tummy tuck may be the best option to address your concerns. Although it involves a longer incision, an extended tummy tuck removes excess, hanging skin from the front (upper and lower abdomen) and the flanks, as well as tightens the abdominal wall.

The resulting scar from an extended tummy tuck can often be hidden by clothing, including many swimsuits, but it may not be entirely covered for some patients. Even so, many patients consider some visible scarring a worthwhile price for a much improved body shape.

Some men find that one or both breasts may be abnormally large. Known as gynaecomastia, this condition can result from a hormone imbalance or from being very overweight, which can increase levels of oestrogen and cause breast tissue to grow. Breast reduction surgery is an option for men with this condition. It may also be used to tighten the breast area after dramatic weight loss that has caused the skin to sag. However, it is not suitable for men whose large breasts are simply excess fat from being overweight.

What does it involve?

Breast reduction surgery is usually carried out under general anaesthetic. There are several techniques the surgeon could use, but generally the operation involves:

making a cut (incision) around the nipple

using liposuction to suck out excess fatty tissue

extending the cuts and repositioning the nipples if there is a lot of tissue to remove

The operation takes about 90 minutes. You usually need to stay in hospital overnight.

Recovery

It can take a few weeks to fully recover from breast reduction surgery. You may need to take a few days off work. It can take up to six months to see the full results. You will need to wear an elastic garment day and night for a few weeks after the operation. The length of time you need to keep the dressings on depends on how quickly your wounds heal. After one or two weeks, your stitches will either dissolve or be removed. You should avoid stretching, strenuous exercise and heavy lifting for up to six weeks after the operation. You can drive again when it’s no longer painful to wear a seatbelt, which may be several weeks.

Side effects to expect

It’s typical after breast reduction surgery to have:

sore breasts for a few weeks

scars – these may take several months to fade

Most men are just left with a scar around the nipple. But a large breast reduction operation may also result in a scar that runs down vertically and horizontally across the breast crease.

Women with sagging breasts, which is called “ptosis”, can also receive breast implants. Depending on how much the breast sags, an additional surgery may also be necessary. This surgery will also add scars all the way around the nipple-areola unlike most breast augmentation surgeries. How do I choose my new size?

This is often one of the most difficult decisions, and for many reasons, has the most potential for disappointment. During your consultation, I will work with you to identify what size of breast you desire from augmentation. “Breast size” as measured by bra-size is variable and is often not a good way to measure final size. As a general rule, every 125-150cc of implant volume equals an increase in a single cup size. However, every patient’s body is different, you can set obtainable and healthy expectations together. It is helpful for you to bring in a bra the size you would like to be to help in finding the right sized implant for you.

Will my nipple sensation or feeling change?

Most women will have loss of some feeling or sensation following breast augmentation dubai  surgery. For most of them, this lasts 6-12 months before returning to normal. 15% of women, however, have permanent alterations in nipple sensation.

How long until I can exercise?

It is recommended that patients begin walking immediately after surgery. However, women should not perform any intense physical exercise for six weeks following the operation. Physical exercise including weight lifting, biking, jogging, and other forms of intense activities may cause implants to shift position or cause wound healing problems that may alter the appearance of the breasts after surgery.

When can I return to work?

This depends on your job and its physical demands. Some general guidelines are: Upper arm movements, like reaching, should be avoided for the first 1-2 weeks. Lifting anything heavier than 5 pounds should be avoided for 6 weeks after the operation. This lifting restriction may prevent some women from returning to work.

How much pain can I expect?

The pain from breast augmentation dubai  surgery is usually in the moderate range and generally can be well controlled with medication in the first 1-2 weeks following the surgery. It is important to note that severe or untreatable pain following surgery can mean infection or another complication.

When can I drive?

Women may start driving a car one week after surgery as long as they are not taking any pain medications.

Will the new implants affect my physical functioning, can I lift heavy objects?

It is unlikely that you will experience long term changes in physical functioning. Depending on the size of the implant, most women find no trouble performing most physical activities following the surgery. Weight lifting or lifting heavy objects will not affect the implant once the area has properly healed.

Can I breast feed after augmentation?

Yes. Placement of the implant below the breast tissue, as in subglandular placement, does not affect the ability of the breast to produce milk. Similarly, submuscular placement, or implant placement below one of the chest muscles preserves proper breast functioning. With the peri-areolar incision, an increased risk of breast-feeding problems may exist. In one study, 7/8 patients reported problems with breast-feeding following peri-areolar breast augmentation surgery. For women who choose the infra-mammary or trans-axillary incision, (incisions under the breast and through the armpit), breastfeeding is usually not a problem. Be sure to discuss your breast feeding needs with your surgeon during your consultation.

Will I need additional surgeries?

Sometimes additional surgeries may be required for breast augmentation dubai  patients. To reach optimal symmetry, you may desire follow up breast lifts or other supporting procedures. Additional surgeries may be needed if you face any complications during your augmentation, like implant rupture or capsular contracture. In addition, implanted breasts will age just like any other body part and in the future some woman choose to have additional operations to correct the results of gravity and aging. There are a variety of abdominoplasty, or tummy tuck, procedures available, and operations can be tailored to suit a patient’s wishes and needs. Generally speaking, however, tummy tucks are most commonly carried out to remove excess skin, unwanted scars, stretch marks and fat, and to tighten the stomach muscles. The ideal patient is someone whose weight is normal, particularly those who have been overweight and have lost the excess weight. Abdominoplasty is not an operation for people who are overweight, or as an attempt at losing weight. In appropriate patients abdominoplasty can have a very marked effect and is associated with high patient satisfaction.

Full abdominoplasty
For patients who have significant skin laxity, excess fat and separation of the muscles, a classic tummy tuck is the most common procedure. Performed under general anaesthetic, this operation can require patients to be in hospital for two or three days.

During the operation, an incision is made from hip to hip and around the umbilicus. The excess skin and fat is excised from the umbilicus to just above the pubic hair. The muscles above and below the umbilicus are tightened. The skin is then sewn up to give a circular scar around the umbilicus and a long scar across the lower abdomen. Although this operation leaves a large scar, it does provide the greatest improvement in abdominal shape. Patients who are thinking about becoming pregnant should not undergo this procedure, and should wait until they are sure they are not having any more children.

Mini abdominoplasty
For patients with only a small amount of excess skin a lesser abdominoplasty might be appropriate. A general anaesthetic is still needed.

During the operating, a wedge of skin and fat is excised from the lower tummy leaving a horizontal scar above the pubic hair. Sometimes the muscles will also be tightened. No scar is left around the umbilicus, which may be stretched slightly to become a different shape. A mini abdominoplasty will give a smaller effect than a full abdominoplasty.

Liposuction
Liposuction can sometimes be done at the same time as the abdominoplasty. However, liposuction can be recommended instead of an abdominoplasty for younger patients who have good quality, elastic skin and whose main problem is a localised excess of fat.

A general anaesthetic is most commonly used, but treatment can often be as a day case. During the procedure, small incisions are made, excess fat is removed and the wounds are then closed. Some degree of skin tightening or shrinkage can be expected as well, but the results of liposuction can sometimes be unpredictable. Liposuction alone will have less effect than abdominoplasty.

Changed the doctor last minute simply because he couldn’t do the surgery the way I wanted it to be done. I wanted the implants under the muscle not above so I found doctor vego who introduced me to third option which is Dual Plane. During my researches I read about this technique that is most used by doctors recently especially in Thailand but I didn’t think I would meet a doctor who would be able to do it for me. What I liked about My doctor is that he gave me all the options plus his own opinion and recommendations and left me the space to choose the incision, implant type and size, and implant placement, etc. He is surrounded by strong team including his assistant who was available 24 hrs to answer my questions and worries and planned everything for me as I only had two days between consultation and surgery. Anastasia doctor was super nice and nurses as well. Supported me and helped me with everything as I came by myself to the hospital to do the surgery and you can imagine how scared and lonely felt. Post surgery: no bleeding, no infection, very very light aching compared to other people’s experiences. Recovery was quick without any complications. After two weeks of free follow up I was able to travel for holiday and I resumed work exactly after 20 days considering that it’s a very physically demanding job yet everything went well.

I would like here not only to give a quick review but to share life time experience with Dr. Vigo. I came to know him long years ago when I decided to go for my liposuction operation in Dubai. After doing some research and checking several options, it didn’t take much time to decide that yes he is the one. I did the operation and results were as per my expectation but his post operation professional approach, the time, the caring and attention he gives to his patients were beyond my expectations. Few years later I went through another plastic surgery (Otoplasty) to fix a defect in my ears shape. The results were amazing and far beyond my expectation, Why? Because it was him again who did it. I always say in a city like Dubai, it is never easy to find honest and experienced doctors and then to make up your mind and decide to go for operation or surgery unless you meet a doctor like Dr. Vigo…and if you do, consider yourself lucky, in safe hands and see real natural results afterwards.
